I am attaching to you two files. The first one is the care plan file that you will use and fill out. The second file contains pictures of the patient I was taking care of. I did not do much that day for the patient. For the medications part use the rarest ones (just five of them). Please make it ESL, very simple English.

2. Adult Calcium Replacement Protocol
* Purpose: Electrolyte replacement.
* Route: IV if calcium levels are less than 0.80.
* Infusion: 2 grams of calcium chloride.
3. Adult Magnesium Replacement Protocol
* Purpose: Electrolyte replacement.
* Route: IV if magnesium levels are less than 1.2 mg/dL.
4. Phosphate Replacement Protocol
* Purpose: Phosphate replacement.
* Route: IV.
* Daily PRN.
5. Potassium Replacement Protocol
* Purpose: Potassium replacement.
* Route: IV.
* Daily PRN, if levels are 3.0 or less.
6. Clopidogrel (Plavix) 75 mg tablet
* Purpose: Antiplatelet agent to prevent blood clots.
* Route: Oral, daily.
7. Dextrose 50% Syringe (12.5–25g)
* Purpose: Treatment for hypoglycemia (low blood sugar).
* Route: IV PRN, when blood sugar is less than 50 mg/dL.
8. Dextrose 15g/32 mL gel
* Purpose: Oral treatment for hypoglycemia (low blood sugar).
* Route: Oral, PRN, when blood sugar is less than 70 mg/dL.
9. Ferrous Sulfate (325 mg)
* Purpose: Iron replacement for anemia.
* Route: Oral, daily, 2 times with meals.
10. Finasteride (Proscar) 5 mg
* Purpose: Treat benign prostatic hyperplasia (BPH).
* Route: Oral, nightly.
11. Fluconazole (Diflucan) 200 mg
* Purpose: Antifungal agent.
* Route: Oral, daily.
12. Glucagon Recombinant Injection (Glucagen)
* Purpose: Treatment for severe hypoglycemia.
* Route: IM (intramuscular) injection, PRN, when blood sugar is less than 70 mg/dL.
13. Hydralazine (Apresoline) 25 mg
* Purpose: Antihypertensive medication.
* Route: Oral, 2 times daily.
14. Insulin Lispro
* Purpose: Blood sugar control for diabetes.
* Route: Subcutaneous injection, sliding scale, 3 times daily.
15. Senna (Senokot) 8.6–50 mg
* Purpose: Laxative for constipation relief.
* Route: Oral, nightly.
16. Tamsulosin (Flomax) 0.4 mg capsule
* Purpose: Treat benign prostatic hyperplasia (BPH).
Route: Oral, every morning.
